COMMUNITY PARTICIPATION SERVICES

Support for connection, skill development, and community engagement.

Community Participation Services (CPS) support adults in building life skills, strengthening relationships, and participating in meaningful community experiences based on their interests and goals.

Participation Activities

Experiences are offered in small groups and provide people with meaningful opportunities to learn, grow, and belong. These supports may include taking part in the community, volunteering, enjoying recreation and wellness activities, and learning everyday life skills such as cooking, budgeting, communication, and building relationships. Activities are person‑directed, led by each person’s own choices, interests, and personal goals. Supports are focused on helping people gain independence, build confidence, and feel a sense of belonging. Services are delivered throughout West Niagara and take place in a mix of centre‑based programs and community‑based locations, allowing people to learn, grow, and connect in ways that work best for them.

Hopkins Community Participation Services

Hopkins Community Participation Services offers structured programming for adults attending from one to five days per week.

4330 Lincoln Avenue, Beamsville
Monday to Friday, 8:00 a.m. – 4:00 p.m.

Programming focuses on life skills, goal planning, wellness, community involvement, and recreational activities in a supportive, person-directed environment.

Adult Case Management

Adult Case Management is a collaborative, person-directed process based on each person’s goals, needs, and priorities.

An Adult Case Management Coordinator (ACMC) works alongside the person and the people important in their life to develop a plan, coordinate services, and connect them with community resources. Support may include education, employment, housing, financial assistance, social opportunities, and other specialized services.

Planning is guided by each person’s active involvement, with clear steps and defined supports to help them move toward meaningful outcomes. Contact: 905-563-4115

Eligibility: Participants must be 18 or older.
